Simple Tips For Home Improvement Jobs TodayRJ Construction ...

Look carefully for home improvement information. It's not always possible to find the exact instructions for every project; however, effective study will enable you to sort out any home improvement challenge. The below article provides excellent information on how to easily make improvements to your home.

Pay attention to the forecast. If you live in an area that is known for strong winds and heavy rains, a patio or barbecue might not be the best investment. Adding a patio cover, or something to ward off the wind can help alleviate the problem. However, if you do not want to spend money on these additional things, it is best to avoid it altogether.

Old toothbrushes can be used to clean your house. It doesn't matter if the mess is located around a large kitchen sink or within the cracks of a bathroom tile, wipe up all the remainder via the detailed bristles on your favorite old toothbrush. The best payoff may be that they are completely free. There is no sense in throwing a toothbrush away when it can be recycled for many useful projects in your home. Instead, use the money you would have spent on a new brush for other things.

Change the doors on your cupboards. Now, not all of us can get new cabinets. Changing the doors can give your cabinet a whole new look though. Be certain to measure properly in advance, though. It is easy to install. All you have to do is screw them in and they're done.

A window or good ventilation needs to be placed in your bathroom. Showering and bathing lead to humidity, which leads to mold. Painting the bathroom is a good way to hide the mold but it will not treat the problem. You need to take precautionary measures to stop it from ever growing. Install a ventilation fan or a window to reduce the humidity in the space.

Don't go over your budget! When you plan on a budget, you ensure that your project is well within your means, but still works for the look of your neighborhood without emptying your wallet. Make a budget, and be sure to stay as close to it as possible until you are finished.

When making home improvements, try going with granite tiles rather than a granite slab. A whole slab can cost in the thousands of dollars. However, with tiles you're looking at a much less cost of only around $300. Granite tiles have the look you want to look at, at a mere fraction of cost.

Do bugs keep you from enjoying the outdoors? The addition of a screened porch can be the perfect solution. Use this area to provide a general sitting-area for yourself and the guests when the group decides to head outdoors. If you would like, you can add a large ceiling fan.

When trying to flip a house, the bathroom is key. Other living spaces, like living rooms and bedrooms, are easily given a new look by the buyers, but a bathroom needs to be in tip top condition. Make sure your tub/shower and sinks are all up to par, and if they aren't, replace them. Swap out your floors for tile if they're linoleum.

If you put new cabinets in your kitchen you should be sure they are securely screwed in the wall. Always find and clearly mark the studs in you walls before installing anything so that you can securely attach things. You can buy a cheap stud finder from a hardware store to make this go more quickly.

An excellent investment for all homes is soundproofing the interior walls. Soundproofing the walls may look beautiful and leave a luxurious feel, but not every wall needs such treatment. The most important rooms to soundproof are bathrooms, bedrooms and equipment rooms. The kitchen can be a nice area to soundproof as well, unless it's a fairly open kitchen that attaches to a lot of different areas.

Use sandpaper when planning to stain or paint any wood surface, and then wipe using a damp rag. Using fine-grit sandpaper is the best way to smooth the surface. Wiping off the object afterwards will keep it smooth and get rid of dust.

Almost every homeowner is going to make home improvements. But not all are successful at it. It's not uncommon for something to be done wrong because someone didn't take the time to learn to do it the correctly. Don't be someone in the status quo. Use the advice in this article to make great, working improvements to your home.

California lawmakers vote to ban gay "conversion" therapy for minors

SACRAMENTO, California (Reuters) - California's state Assembly approved a bill on Tuesday to prohibit children and teenagers from undergoing a controversial therapy that aims to reverse homosexuality, moving the state closer to becoming the first to impose such a ban.

The 51-21 vote in the Democratic-controlled Assembly marked a major victory for gay rights advocates who say the so-called conversion therapy has no medical basis because homosexuality is not a disorder.

Opponents of the practice say such attempts to change sexual orientation can cause depression and lead to substance abuse and suicide.

Several openly gay legislators championed the bill in emotional speeches during the floor debate, citing their own childhood experiences.

"One of our No. 1 priorities in this house is to protect the next generation of Californians," said Ricardo Lara, a Democrat from Los Angeles County. "And some of those are sissy boys. And some of those sissy boys grow up to be Assembly members. And some of those sissy boys need help. And we are here to stand with those sissy boys."

The Senate passed its version of the bill by a vote of 23-13 in May. Lawmakers must iron out minor differences in the two measures by Friday before a final bill makes its way to the desk of Democratic Governor Jerry Brown.

Brown has not indicated whether he supports the measure, and a spokesman for the governor said that he would not comment on pending legislation.

Assembly member John Perez, the first openly gay speaker of the California's lower house, said during floor debate that "it is inappropriate for anyone, including parents, to subject anybody to dehumanizing activity," referring to the therapy.

The bill's sponsor, state Senator Ted Lieu, said in a statement that the psychiatrist who pioneered such therapy, Dr. Robert Spitzer, has since renounced it and apologized to the gay and lesbian community.

If Brown signs the bill into law, California would become the first state in the nation to outlaw such therapy for minors.

Republican opponents of the measure said politicians should not step in and regulate what they consider to be a matter for medical boards to decide. They also said the bill encroaches on the rights of parents to make choices for their children.

"That's why parents have children -- to hand down their legacies, their belief systems, the way they want their children raised," Assembly member Shannon Grove, a Republican from Bakersfield, said during the floor debate.

Weight Loss After Menopause

? Fitness and Nutrition ? ? Diets and Dieting

Information provided by Elsevier Health Sciences - Published: 2012-08-28

Is long-term weight loss possible after menopause? Strategies for success discussed in Journal of the Academy of Nutrition and Dietetics.

Many people can drop pounds quickly in the early phases of a diet, but studies have found that it is difficult to keep the weight off in the long term. For post-menopausal women, natural declines in energy expenditure could make long-term weight loss even more challenging.

A new study finds that in post-menopausal women, some behaviors that are related to weight loss in the short term are not effective or sustainable for the long term. Interventions targeting these behaviors could improve long-term obesity treatment outcomes. The research is published in the September issue of Journal of the Academy of Nutrition and Dietetics.

Lead investigator Bethany Barone Gibbs, PhD, University of Pittsburgh Department of Health and Physical Activity, explains that a number of factors work against long-term weight loss. "Not only does motivation decrease after you start losing weight, there are physiological changes, including a decreased resting metabolic rate. Appetite-related hormones increase. Researchers studying the brain are now finding that you have enhanced rewards and increased motivation to eat when you've lost weight." Combined with the natural energy expenditure decline in women following menopause, it is extremely difficult for older women to lose weight and maintain weight loss.

Traditional behavioral treatments for obesity, focused on caloric intake, have had poor long-term results.

The investigators sought to determine if changes in eating behaviors and selected foods were associated with weight loss at six and 48 months in a group of overweight post-menopausal women. A total of 508 women were randomized to either a Lifestyle Change group or a Health Education group. The Lifestyle Change group met regularly with nutritionists, exercise physiologists, and psychologists throughout the study. Their goals were to reduce fats and caloric intake, increase consumption of fruit, vegetables, and whole grains, and participate in regular moderate exercise. The Health Education Group was offered seminars by health professionals on general women's health, but not specifically weight loss.

Investigators found that the eating behaviors associated with weight loss at six months were eating fewer desserts and fried foods, drinking fewer sugar-sweetened beverages, eating more fish, and eating at restaurants less. After four years, they found that eating fewer deserts and drinking fewer sugar-sweetened beverages continued to be associated with weight loss or maintenance. Eating fewer desserts and drinking fewer sugar-sweetened beverages were also related to long-term weight loss. However, eating more fruits and vegetables and less meat and cheese emerged as additional important predictors for long-term weight loss. Eating at restaurants declined at 48 months whether or not subjects lost weight. Dr. Barone Gibbs speculates that this may have been related to economic factors and not relevant to the study.

Dr. Barone Gibbs explains that strategies such as eating fewer fried foods may not be sustainable for the long term.

"People are so motivated when they start a weight loss program. You can say, 'I'm never going to eat another piece of pie,' and you see the pounds coming off. Eating fruits and vegetables may not make as big a difference in your caloric intake. But that small change can build up and give you a better long-term result, because it's not as hard to do as giving up French fries forever."

The results suggest that decreased consumption of desserts and sugar-sweetened beverages consistently associate with short- and long-term weight loss or maintenance, but increased fruits and vegetables and decreased meat and cheeses are additional factors that may improve long-term weight loss or control. "If the goal is to reduce the burden of obesity, the focus must be on long-term strategies because changes in eating behaviors only associated with short-term weight loss are likely to be ineffective and unsustainable," concludes Dr. Barone Gibbs.

In an audio podcast accompanying the study, Dr. Barone Gibbs discusses the results in depth, including its implications for men and women of all ages and the challenges of self-reporting in research on diets and eating behaviors.

IDriveSync

How do you keep all your files within easy reach so that, no matter what device you're using at the moment, you can always pull up that important document when you need it? The answer, for many people, is a file-syncing service, and the one you choose may depend largely on how much space you need. IDriveSync (free to $149 per year for 500GB) may be a lesser-known offering, but it offers a lot of storage space for comparatively little money.

Two of the most well-known services Dropbox and SugarSync?both Editors' Choices at PCMag?are more elegant to use, but cost more when you pay for a plan with additional storage. Each of these three file-syncing services has a free version, as well, and I break out all the pricing scenarios and free space allotments on page 2 of this review.

For more services, see "The Best File-Syncing Services."

The Basics
IDriveSync is a free file-syncing program. Like most other file-syncing services, IDrive installs locally on any device where you want to access your files: computers, smartphones, and tablets. On your computers?yes, plural?the app either creates a new folder or asks you to select existing folders on your machine that will be synced. The default location on Windows is:?

Documents and Settings\username\IDriveSync.

Once you've set up the program, all files housed in that folder will back up to the file-syncing service's servers, and when you retrieve those files through your IDrive apps, such as the Android app, Web app, or iOS app, you'll always have the most recent version of the file automatically.

IDrive lets you store all kinds of file types, and, to make sure you understand that point, it establishes sub-folders automatically on installation for Documents, Music, Photos, and Video. An instructional PDF called Getting Started and a few sample files are also included.

After the app is installed, the quickest way to access settings and see how much of the allotted storage space you've used is through the pop-up menu from the system tray in Windows (see the slideshow) or top menu bar in Mac. From time to time, I encounter system tray menus that I couldn't read even if I had a magnifying glasses, but IDrive's uses an appropriate font and leaves ample room between text lines to actually be able to see what options exist.

Launching the Preferences window lets you get your hands on a few of the more advanced settings and features, like changing the location of the IDriveSync folder, pulling up activity logs, setting up proxy settings, and adjusting the bandwidth usage. The bandwidth settings aren't clearly labeled, however, until you hover over the slider bar and their full description pops up, so it's not clear what they do until you start to change them, a big no-no, because inexperienced users know better than to muck around with controls unless they understand what they do. (For the record, the two sliders are housed under "Network" and labeled "User Active" and "User Inactive," and both are set to 100 percent by default. Hover over the button to change them, and the pop-up descriptions say, "Select the bandwidth to be used when the system is in use/not in use.")

IDriveSync in Use
Using IDrive is mostly seamless because it works in the background quietly. Visual cues let you know the app is getting its job done. Files with a green circle and check mark indicated that they're synced, while a blue circle with white "recycle" lines indicates that syncing is in progress. These same icon conventions are used in most other file-syncing programs, too, so it's par for the course.

One feature I always like to see in file-syncing services is the ability to "right-click to sync" on any file, anywhere on your system. SugarSync set the bar for me with this feature, and IDrive tried to instate it, too, but to much less satisfactory results. When you right-click a file and choose to save it to an IDrive Sync folder, the system actually makes a copy of it and sends that copy to the IDrive folder, so your original file remains intact. The file you've now chosen to sync is completely out of sync because there are two versions of it! That's a huge, glaring problem, because it's counter-intuitive to what file-syncing services do, although the company behind IDriveSync assured me that it is what they intended.

Dogs Prefer This Music Most of All | Babble Pets

dog headphones 300x199 Dogs Prefer This Music Most of All

Wonder what this pup is listening to?

If you have kids, you?ve undoubtedly heard about providing the right kind of music for your baby. Some moms even try to foster their child?s music appreciation in utero by placing headphones on their belly while pregnant. When I was pregnant, I didn?t do that but I did listen to what I liked (which translated into lots and lots of Dave Matthews). But my tastes are varied so I also listened to everything from REM and System of a Down.

Naturally, my dogs have listened to the same. But do dogs have any one favorite type of music?

Researchers believe they do?.but only to a certain extent.

There are so many?ways?in which dogs exhibit similar behavior to people, and their basic music preference may be one of them. Dogs tend to prefer classical music according to a study on shelter dogs performed by Dr. Deborah Wells, a psychologist and animal behaviorist:

The shelter dogs reacted most positively to classical music. They found it soothing and showed fewer signs of stress when they were listening to classical music. They rested more and barked less, she says, and behaved with less of the agitation well known to those of us who volunteer in shelters.

Dr. Wells also found that the worst music for dogs was (you guessed it) heavy metal. Pop music and the sound of human conversation had no effect on dogs, probably because dogs are so used to both of those things.

The study does suggest, however that dogs do have some individual preferences, and I suspect those tend to tie in with their owners preferences.

My kids tell me now that they always get a good feeling when they hear a song I?ve played a million times when they were little. I hope my dogs do. too!

What music does your dog listen to? What do you think he/she likes the most?

San Francisco archbishop-elect in DUI arrest

SAN DIEGO (AP) ? The Roman Catholic archbishop-elect of San Francisco is apologizing for his weekend arrest on suspicion of drunken driving.

The Rev. Salvatore Cordileone said in a statement issued by his office Monday that he was driving home from a dinner with friends in San Diego with his mother early Saturday when he was pulled over at a DUI checkpoint. He said a sobriety test showed his blood alcohol level to be above the legal limit.

Cordileone currently serves as bishop of Oakland and is scheduled to be installed as San Francisco archbishop on Oct. 4, five days before his first court date.

He says he is ashamed of what he termed an "error in judgment" and plans to pay his debt to society.

Why Does Customer Service Matter? - Business Consulting Firm

Most companies want to increase the quality of their customer service ? do you? Why do you?

Why do I ask?

Well, here?s the deal. Given most companies want to deliver better customer service, it seems like the right thing to want to do the same, right? I mean, imagine the response to a CEO who declares, ?No! We?re not interested in delivering better customer service!? The reaction probably wouldn?t be positive. An interest in delivering better customer service seems like the right thing to do ? but having good motives really does no good at all, unless you know why you?re motivated to change.

Defining and identifying the ?why? will do wonders for your efforts.
? What about your current customer service is responsible for the lack of results you are experiencing?
? What aspect of great customer service would give your company ?the biggest bang for its buck??
? In what ways is the quality of your current customer service interfering with your ability to accomplish your ideal business goals?

All in all, why are you driven to improve your customer service?

When I?m consulting with companies over this question, they typically answer this question by regurgitating stats that I?ve given them. They know the bottom-line benefits of customer service improvements, so it?s easy for them to know why it?s important?but that isn?t really what I?m asking. I?m asking a much more personal question that should elicit a much more personalized answer. In what ways will improving customer service at your company give your company a better chance at reaching the results your company aims to achieve?

Do you get it?

Your answer should be unique to your company?s situation. If everyone is simply inspired to deliver better customer service, so that their company?s bottom line improves?well, that?s too ambiguous. How can your employees take a stake in its outcome? How can you tangibly measure the progress? How can you know whether you?re doing well on accord of your own standards or simply surpassing competitors?

You can?t.

You can?t give your employees a reason and a way to focus their efforts, you can?t measure anything concretely and you can?t assess your progress without clarification from the beginning. Knowing great customer service influences bottom-line results is a very different scenario than knowing how customer service improvements will drive bottom-line increases. In the first case, you understand they are connected ? in the second case, you know why they are connected. And if you know why, you can take action.

I?ll conclude with an example to drive this point home. The Holiday Inn and the Waldorf Astoria could both want to improve customer service, but why and how they plan to do this should look dramatically different given the Holiday Inn is a standard hotel chain, whereas the Waldorf Astoria is a five-star, luxury hotel. Both can still achieve their goals because their audience and hotel experience vary greatly, but if the Holiday Inn only knows that it needs to improve its customer service because other hotel chains are?well, the Holiday Inn doesn?t have much direction or authentic investment behind the effort. But if the Holiday Inn knows that its usual customer is NOT the typical Waldorf Astoria customer, well then, it can start to determine what the Holiday Inn customer wants, needs and expects from the Holiday Inn. It can start to define goals, priorities and problems within the Holiday Inn context, rather than attending to every possible thing any customer of any hotel chain could need, want and expect from any other hotel chain.

Knowing why customer service is important to your company may seem like an intangible piece of information to identify and define, but in the long run, understanding this detail makes your efforts much more focused, streamlined and fruitful. Dig into why customer service improvements matter to you, so that your efforts are purposeful and prosperous.

California Exchange Moves to Demystify Health Insurance for ...

Many important topics were on the agenda during yesterday?s California Health Benefit Exchange Board meeting.? RemakingCalifornia?s health insurance marketplace to be ready to start enrolling people in October 2013 is no small task.? The Exchange?s goal is to create a first class consumer experience for buying and using health insurance, a quantum leap from what consumers experience in today?s dysfunctional marketplace.??

For decades big insurance companies have written the rules about how they would market their products.? Consumers were forced to make apples-to-lemons comparisons, and companies refused to cover people with pre-existing conditions or dropped coverage when people became ill.?? The result: record insurance company profits, increased consumer exposure to financial harm and illness, and skyrocketing system costs, since many people?denied coverage turned to charity care.

Yesterday the Exchange began to rewrite the rules with consumers in mind.? Taking public input from Consumers Union and many other stakeholders representing health care consumers, they voted to set the terms for insurers in the Exchange who must offering essential health benefits, including maternity care, hospitalization, primary and emergency care, among other benefits. For a full list, click here.

In the new health insurance marketplace, standardization of health plan offerings is key. Consumers must be able to make apples-to apples comparisons of what benefits are, how much will it cost, and whether the provider network is adequate to meet their needs. ?Insurers will be competing in a functioning marketplace based on cost, quality and network size for millions of customers recieving premium assistance and help with co-pays via the exchange?as many as 2 million of whom are currently uninsured.

Yesterday, the Board also voted re-elect Diana Dooley, Secretary of the California Health and Human Services Agency, as the Exchange Board Chair.? Diana is a committed leader and passionate about makingCalifornia?s Health Benefits Exchange the best in the nation. We congratulate her on her new term and look forward to working with her over the coming year!

Finally, the Board?s Executive Director Peter Lee announced that the California Exchange was approved for $196 million federal grant for implementation through 2014.? This means the Exchange can move full steam ahead on making our health insurance marketplace work for all Californians.

All at sea: a history of maritime communications | British Science ...

Titanic's Marconi apparatus

Titanic?s Marconi apparatus

Communication is something that has been around since the dawn of life. Animals and plants use many different methods of communicating with each other, using sounds, smells and other signals.

Before the discovery of electromagnetic fields and electricity, humans communicated using direct speech, semaphore or hand signals. Semaphore signalling, devised by Lord George Murray, uses six shutters, two vertical rows of three ? on a white background. By changing the pattern of the black and white stripes of the shutters, words and sentences could be spelled out to communicate between ships.

This was all very well if you were within sight of each other, but what happened when one of you was beyond the horizon? This was a problem that the British Royal Navy was determined to overcome. Communicating between ships, and from ship to shore is vital for survival on the seven seas.

To overcome the longer distances they had to be a little more creative. In the late 1800s the Royal Navy started to work with the ideas from Heinrich Hertz on electromagnetic radiation, and work with scientists such as Guglielmo Marconi to use radio waves as a method of communicating between ships and from ship-to-shore in Morse code.

In the mid-1800s, James Clerk Maxwell, a well-respected physicist and mathematician developed his theory on electromagnetism. He showed that light waves consisted of oscillating, perpendicular electric and magnetic fields.

By converting sound waves into radio waves, ships could send information across vast stretches of ocean to communicate with their allies. They would then decode the radio waves back into sound waves, and listen to the message they had received. By the early 20th Century, the Royal Navy had started to adopt transmitters fitted with alternators and used magnetic detectors with headphones.

After the discovery of microwaves, the U.S. Navy realised that they could bounce these waves off the surface of the Moon, the Earth?s natural satellite. This would allow them to send information to the other side of the world with much less interference. After the USSR sent Sputnik, the first artificial satellite to contain a radio transmitter up into space in 1957, the US followed soon after. This ?space-race? led to more efficient satellite communication systems. Since then, technology has advanced at an impressive rate, improving the way in which the Forces can communicate with each other.

Professor Tom Stevenson, who is an expert in microfabricating integrated circuits (chips that are essential in modern communication technologies) is one of the trustees of the Museum of Communication which opened in the 1970s. The museum features methods of communication ranging from the pre-electric optical telegraph used during the Napoleonic Wars to modern webcams and cameras for mobile phones.

Very often, cutting edge research of the time will be funded and used by the military, leading to huge strides in the development of new technologies. Prof Stevenson?s talk, ?All at sea: the history of maritime communication? will look at the underlying physical developments that have allowed for such leaps in communications technology.

Join Prof Stevenson and listen to his talk on Thursday September 6 from 14:00 ? 15:00 in the Meston Building.

Should I pursue a long distance relationship with a friend I've been ...

7 hours apart, by car or by plane. That makes a difference. In reality, unless it's a temporary situation, the chance of it working out are realistically slim to none. The only time that much distance works is if it's temporary, and/or you're engaged or married. Relationships take physical contact, intimacy, face to face contact, even something as small as just holding each others' hands. Unless you're already committed to each other, it's not going to work out. You can't build a relationship over the internet, texting and phone. It just doesn't work. Find someone closer to you.

Complete Guidelines to Health and Fitness: ICU Nursing-Nurse ...

ICU Nursing

Intensive Care Unit (ICU) nursing is commonly referred to as critical care nursing. Critical care nursing deals specifically with the human response to life threatening conditions. Critical care nursing is challenging due to the life-threatening health situations in the ICU. Critical care nurses are often in highstress situations which demands complex assessments, highintensity therapies and interventions and continuous vigilance.

Acute Exacerbations of COPD

Chronic obstructive pulmonary disease (COPD), also known as chronic obstructive lung disease (COLD), is a term used to describe progressive lung diseases, which include emphysema, chronic bronchitis and chronic asthma. The common symptoms of COPD are progressive limitations of the airflow into and out of the lungs and shortness of breath. Emphysema and chronic bronchitis are closely related and patients with COPD may have both, which affects lung function. Emphysema involves destruction of the alveoli in the lungs. Chronic bronchitis is characterized by chronic cough and mucus production.

Over a period of time the patient experiences abnormal ventilationperfusion, insufficient oxygenation of blood (hypoxemia), hypoventilation and right-sided heart failure. People with COPD have a variety of illnesses such as, atelectasis which occurs due to the collapse of part or all of a lung by blockage of the bronchus or bronchioles or by very shallow breathing; bronchiectasis, which is an acquired disorder of the large bronchi that become dilated due to destructive infections of the lungs; congestive heart failure (CHF),a disorder in which the heart loses its ability to pump and cor pulmonale ,where the right ventricle gets enlarged because of pulmonary hypertension from lung disorders. COPD symptoms, when ignored, usually lead to hospitalization in intensive care (ICU) units.

Nurse Interventions in Acute Exacerbations

People with chest deformities or neurologic conditions that cause shallow breathing benefit from mechanical devices that assist breathing, such as continuous positive airway pressure, which delivers oxygen through a nose or face mask that prevent airways collapse, even at the end of a breath. Additional respiratory support can be provided with a mechanical ventilator. The primary treatment for acute massive atelectasis, a common complication in COPD is removal of the underlying cause (Brooks-Brunn, 1995).

If the blockage cannot be removed by coughing or by suctioning the airways then it should be removed by bronchoscopy. Antibiotics are to be given for any detected infection as in chronic atelectasis, when infection is almost inevitable. Treatment of atelectasis due to deficient or ineffective surfactant is done by treating the low blood oxygen either with mechanical ventilation or positive end expiratory pressure. For cor pulmonale, supplemental oxygen can be administered to increase the level of oxygen in the blood. A low salt diet is recommended. Diuretics are given to remove excess fluid from the body. Calcium channel blockers, intravenous prostacyclin, or the oral medication bosentan are frequently used to treat pulmonary hypertension. Blood thinning anticoagulants are also useful. Oxygen administration relieves symptoms and prolongs survival. Careful intervention is essential because progressive pulmonary hypertension and cor pulmonale often leads to severe fluid retention, lifethreatening shortness of breath, shock, and death.

Benzodiazepines are not recommended to relieve anxiety in patients with COPD because they decrease respiratory drive and compromise lung function (Brooks-Brunn, 1995). An anxiolytic, buspirone, have been found to be safe in reducing anxiety in COPD patients. Dyspnea is common in individuals with chronic obstructive pulmonary disease. Respiratory assessment of the patient should include present level of dyspnea measured using a quantitative scale such as a visual analogue or numeric rating scale. Usual dyspnea is measured using a quantitative scale such as the Medical Research Council (MRC) Dyspnea Scale.

The other assessments include Vital signs, pulse oximetry, chest auscultation, chest wall movement and shape/abnormalities, presence of peripheral edema, accessory muscle use, presence of cough and/or sputum, ability to complete a full sentence and the level of consciousness. By doing so, nurses should be able to detect stable and unstable dyspnea and acute respiratory failure (American Thoracic Society, 1998). Nurses should also be able to offer interventions for all levels of dyspnea including acute episodes of respiratory distress which includes acceptance of patients' self-report of present level of dyspnea, medications, controlled oxygen therapy, secretion clearance strategies, noninvasive and invasive ventilation modalities, energy conserving strategies, relaxation techniques, nutritional strategies and breathing retraining strategies. It is important for the nurses to remain with patients during episodes of acute respiratory distress. Nurses have to assess patients for hypoxemia/hypoxia and administer appropriate oxygen therapy for individuals for all levels of dyspnea. Medications include bronchodilators, beta 2 agonists, anticholinergics and methylxanthines, corticosteroids, antibiotics, psychotropics and opioids (www.guidelines.gov).

Patient safety checks

Patient safety checks include circuit leaks; maintenance of positive pressure; adequate inspiratory air flow and not leaving the patient alone. Continuous Positive Airway Pressure Oxygen therapy is part of any ICU and requires absolute attention.

Managing the therapy involves maintenance of the desired FIO2; level of positive airway pressure and time period for CPAP therapy, attaching CPAP machine medical air and oxygen gas lines to wall sources, preparation of humidification source ,selection of prescribed FIO2 on oxygen blender, turning flow on to level above 25 litres / min., positioning of rubber securing band behind the patient's head, centred on occiput, positioning of face mask over the patient, adjusting the level of positive expiratory pressure to prescribed level, adjusting inspiratory gas flow so that minimal fluctuations are present on pressure gauge, observing and documenting respiratory rate; work of breathing and SpO2, increasing inspiratory flow if respiratory work is excessive or the patient complains of continuing dyspnea, maintaining continuous SpO2 monitoring with alarm function in place, maintaining humidification temperature at 36 degree C or at temperature tolerated by the patient (American Thoracic Society, 1998).

Patient observations include, visual check every half an hour, documentation of respiratory rate, SpO2, nausea and vomiting, monitoring pulse rate and rhythm; blood pressure; peripheral circulation and proper functioning of humidification system every hour, checking the condition of skin around and under mask and rubber securing band, documentation of condition and interventions, condition of conjunctivae every two hours, auscultation of lungs for equal air entry and palpitation of abdomen for distension every four hours (Vollman,1997). Ventilator-Associated Pneumonia is a common nosocomial infection in the ICU accounting for 13% to 18% of all nosocomial infections (Rello et.al, 1996).

Infection may be even due to improper hand washing, not changing the gloves from patient to patient, and contamination of respiratory devices like nebulizers, spirometers, oxygen sensors, bag-valve mask devices, and suction catheters (Shelby Hixson, 1998). Oral care includes brushing the patient's teeth, use of solutions and mouthwash to cleanse the mouth, and periodical suctioning of oral secretions. Nasal care and proper cleansing of the nasopharynx reduces bacterial infection.

Conclusion

The ICU setting demands stressful nursing interventions and constant monitoring of the patients especially with conditions like COPD. Nurse interventions should be based on assessment of dyspnea, vital signs, pulse oximetry, chest auscultation, chest wall movement and presence of peripheral edema, cough and/or sputum, ability to complete a full sentence and the level of consciousness. Proper oral and nasal care reduces lung infection.

Acai counteracts oxidative stress, lengthens lifespan in fruit flies

ScienceDaily (Aug. 21, 2012) ? Bewildered by the array of antioxidant fruit juices on display in the supermarket and the promises they make? To sort out the antioxidant properties of fruits and berries, scientists at Emory University School of Medicine turned to fruit flies for help.

They found that a commercially available acai berry product can lengthen the lives of fruit flies, when the flies' lives are made short through additional oxidative stress. Under certain conditions (a simple sugar diet) acai supplementation could triple flies' lifespans, from eight to 24 days. Acai could also counteract the neurotoxic effects of the herbicide paraquat on the flies.

The results were recently published by the journal Experimental Gerontology, which awarded the paper its inaugural "Outstanding paper" prize. The lead author is Alysia Vrailas-Mortimer, a postdoctoral fellow in Emory University School of Medicine's Department of Cell Biology.

Vrailas-Mortimer says she didn't start out focusing on acai. But acai worked better than several other antioxidant products such as vitamins, coenzyme Q10 and lutein.

"One thing that makes our work distinctive is that we tried commercially available supplements," she says. "We went to a health food store and filled up a basket."

She says she began the project with the help of undergraduate student Rosy Gomez, and narrowed her focus after initial success with acai. Vrailas-Mortimer took advantage of a discovery she had made working with Subhabrata Sanyal, assistant professor of cell biology, PhD. They had previously found that flies with mutations in the "p38 MAP kinase" gene have shorter lives and are more sensitive to heat, food deprivation and oxidative stress.

P38 mutant flies lived an average of only eight days when they were given a simple sugar water diet. However, their lifespans tripled when their diet was supplemented with acai. Ginger was used as a control for the diet supplements.

Acai also protected normal flies against oxidative stress, in the form of hydrogen peroxide or paraquat. Acai can protect against oxidative stress when flies are exposed to hydrogen peroxide before being given acai, but the protective effect does not hold up if the order is reversed.

Paraquat is an herbicide that has neurotoxic effects that resemble Parkinson's disease. Under the influence of paraquat, flies' sleep-wake cycles gradually become chaotic (see graph). Acai can also help soften the effects of paraquat on flies' circadian rhythms.

"I think this is important," Vrailas-Mortimer says. "We show that whatever is in acai that is lengthening lifespan, it can also keep the flies functioning better for longer when faced with paraquat exposure. It is maintaining quality of life rather than just preventing them from dying."

When flies were fed a more enriched diet of a standard cornmeal/molasses mush, the effects of supplementation with acai were more pronounced in males than in females. Males' lifespans were almost doubled with acai (20 to 40 days) but the effects on females were not as strong (30 to 34 days). On an enriched diet, male flies were more sensitive to paraquat than females as well.

Implications for human clinical trials of antioxidants

Large clinical trials studying the effects of antioxidants such as vitamin C and E have not shown clear benefits on human health. Using fruit flies under oxidative stress as a model can be a way to dissect which components of acai are beneficial. Probing the effects on flies' circadian rhythms can be a way to quickly screen several compounds or regimens, since the effects are apparent even before most of the flies die.

Acai berries contain a variety of antioxidant and anti-inflammatory compounds, such as anthocyanins. Sanyal says it may be better to study the components of acai together rather than in isolation.

"There may be a combinatorial effect, and if you separate the components from each other, you may lose the active principle," he says. "In addition, it seems to me that anti-oxidant therapy will not work after the damage has been done. So human clinical trials that don't take this into account are likely to have disappointing results."

Tips In Choosing A College Or University

One of the most important decisions that a young adult will ever have to make is choosing which college they will go to. Since college can last for four years or more, and can have a lasting impact on your life, choosing the right one for you is extremely important. When going through the process of choosing a college to go to, there are a few different tips that you should follow.

When trying to choose a new college, the first tip is to find a college that offers a good program that you are interested in. While a college may have a strong overall reputation, it does not mean that it offers certain degree programs or has a strong reputation in certain areas. For example, if you are hoping to pursue a bachelor of communication or a bachelor of biomedical science, you should research schools and choose one that offers the program and has a strong reputation in the professional industries. Be sure to consider various other factors such as job placement rates and average salaries of graduates when you are researching jobs.

Another tip that you should follow when choosing a school is choosing a place that offers a lifestyle that you are comfortable with. The campus lifestyle at a college will vary significant from one place to the next. While one campus may offer a very competitive educational oriented environment, another may offer a more laid back environment. Be sure to read reviews of the school to get an understanding of what it is like to live on campus and also take a weekend to visit the campus as well. This will allow you to better understand if it offers an environment in which you would like to live in.

The third tip to follow when choosing a college is to consider what tuition assistance options are available. The price of college has continued to increase faster than the rate of inflation. This means that it is less affordable than it ever was before. Therefore, more people will need to rely on student loans and scholarships than ever before. To ensure that the school you are going to attend is affordable, be sure to get an understanding of what forms of tuition assistance are available. This could include whether there are scholarships and grants available, or if the school offers increased compensation for working a school-related job if the paychecks go directly towards tuition payments. One of the most important decisions that a young adult will ever have to make is choosing which college they will go to.
